You must provide a SSN when applying for U.S. Passport

Please note that you are required to provide your Social Security Number (SSN) when applying for or renewing a U.S. passport. A passport application may be denied if you fail to provide a valid SSN (if you were issued one) or wilfully, intentionally, negligently or recklessly provide incorrect or invalid SSN.

If you have never been issued a SSN, you must enter NONE in box #5 of the application form. Print and fill out the sworn statement. Attach it to your passport application packet.
